How is the total calculated?

Subtotal minus discount gives the taxable amount; tax is applied to that amount; the total is the taxable amount plus tax.

Do I have to add sales tax?

Only if you charge it. Leave the tax field empty if your product or service is exempt or tax is not itemized.
